A complaint allegation was received on 2/16/2024 alleging that AG was bitten on the left ear while he was playing in the classroom. The school called the parents and advised them they had no idea of what happened. When the parents arrived they found the son was bleeding and a teacher was holding gauze on the child's ear. Parents took the child to the hospital where he received 4 stiches. The teacher in classroom was not paying attention to the children because she was on her cell phone talking for 2 hrs. A complaint inspection was conducted on 2/21/2024 and a statement was received from the acting director (EP) and video footage of incident observed. Per the acting director, they asked the child what happened and he said that another child bit him. The video footage of the incident was reviewed and the following was observed; The child (AG) was observed fighting with another child for a toy. The other child threw himself over AG and then pulled away. AG was then observed holding his ear and crying. At the time of the incident the teacher was behind the diapering table organizing the cabinet under the diapering table and on her phone on multiple occasions. She did not attend to the child after he cried nor look in the direction of the child. A few minutes later, she was seen changing another child's diaper. After changing a child's diaper, AG approached her and shared a few words with her and she pointed away as if telling the child to clean up. The child was then observed picking up toys and putting them on the furniture. The child approached the teacher a second time and pointed at his ear. That is when the teacher noticed the wound on the child's ear. The incident happened at 10:53AM and the did not attend to the child until 11:03AM. This complaint is Substantiated and a lack of direct supervision class 1 non-compliance and notice of violation is being issued

The following areas were observed in need of repair during today's visit: • Fix toilet located between the 2 year old classroom observed not working. • Replace electrical cover observed broken in the three year old classroom (room closer to the main entrance). • Fix laminated floor in the 4 year old classroom. (provider has placed a table on top to avoid any tripping accidents) • Ceiling tiles were observed with water stains in the 4 year old classroom. Provider stated the roof has been already repair and they are still pending or repairing the wet tiles. • Clean tire holders observed with black built up dirt located in the playground area. • Replace sand table cover observed broken in the 3 ear old classroom. Ensure sand is not on the floor to prevent any tripping hazards

10-09 · Child Discipline: DCF Handbook
COMPLAINT: Program self reported a staff member that did not follow the discipline policy stablished by the program. INVESTIGATION: On 8/3/23 the school administrator contacted CCLE to self report an incident that occurred on 8/2/23. According to the school administrator, one staff member grab a toddler by the arm when the child was grabbing another child's shirt. The instructor that witness the action stated that the arm grab was harsh. Even thought he child did not have any injuries, the school administrator decided to terminate the staff member since her action was not part of the school policy. The program contacted the parents and created an incident report to document the occurrence. Based on the information provided, the allegation is deemed substantiated
