When the monitor asked the teacher (M) about the empty mat on the floor she verified the child was alone in the infant classroom's bathroom. *During today's inspection monitor observed a afternoon dismissal where multiple children were roaming the facility, some exiting onto the playground. Monitor observed 3 children left behind alone playing in the dramatic play area of the three year old classroom. Monitor stood with the children till the staff returned approximately a few minutes later. There shall be at least one (1) staff person providing direct supervision at all times when one (1) or more children are present. Therefore a Notice of violation will be issued. The Notice of Violation occurred on 2/27/2024. The Notice of Violation was provided to the facility on 3/28/2024

A complaint allegation was received on 5/31/23 alleging that last week, on Tuesday, May 23, 2023 a child had his 15 months check up at the doctor's office and was dropped off at the daycare afterwards. However, mother was told by the owner to remain in car and she would let mother know when to enter. The director did not have time to explain she indicated that she was having an inspection and after the inspection was over she could enter with her son. After 15 minutes of waiting mother texted Director and asked if she could drop son off because she had to go to work. Director replied to leave they are not accepting her son that day. Mother was expecting daycare for the day. There were other children not allow to enter due to the ratio of staff to children. A complaint inspection was conducted on 6/9/2023 and a statement from the owner (SI) was received. Per owner (SI), the mother brought the to the door at around 11:30AM and the child was sleeping in her arms. She refused to accept the child because she was concerned that the child was sleeping after a doctor's visit. The school does not receive sleeping children and especially after 10AM because it disrupts the environment and schedule of a class. The allegation indicated that there is insufficient staff to meet ratio. A walkthrough of the facility revealed the infant class and toddler class to be out of ratio. Upon arrival, I rang the doorbell and looked through the door window, I observed the owner looking at the door standing inside the office on the right side of the building. She was in the office with 2 school age children and husband (M). The owner (SI) then proceeded to open the door. I informed who I was and the purpose of my visit. She proceeded to the infant class and returned with the director. I walked into the infant class with the director and and counted the following ratio; infants 1:8, three infants and five 1y/o children 1y/o and 2y/0 1:9, two out of the nine children were 1-year of age 2y/o and 3y/o 1:7 4y/o and up 1:8 This complaint is substantiated for ratio as there was only one teacher observed in the mixed group of 1s and 2s with 9 children. The infant group was also out of ratio as the director (M) was covering both the 4y/o class and the infant class upon arrival. The school alleges that the owner's husband (MI) was in the toddler class before he went to cover the 4y/o class. He was initially observed in the office and as I was conducting the walkthrough he was observed covering the 4y/o class. When I asked who was with the 4y/o class before (MI) entered the room, The director (M) said she was. When asked who was the second teacher in the infant class when I arrived, she said she was standing in the doorway in between the 4y/o and infant classroom covering ratio for both because the owner (SI) went to open the door. TA was provided that she (M) does not count as a second person in the infant room as she had her own class to supervise and she must be in the infant class only to be counted toward ratio in the room. The owner (SI) did not leave the infant room to open the door as she was already standing inside the school office when I arrived. The complaint is unsubstantiated for what occurred on 5/23/2023 but substantiated for ratio. The school was observed to be out of ratio during inspection
