Per providers statement, the youngest child transported is 6 years old
During todays visit, provider was not able to show proof of a current vehicle inspection. Provider requested a copy from the insurance company, which was submitted to the license monitor via email. This area was corrected the day of the inspection

Complaint alleges that a teacher got bitten by a dog on Friday during pet week. Child is terrified by what he/she says they saw. A dog was inside the school. There is concern about vaccination and allergies that children may have and be exposed to. Parents are not notified or asked for a written consent. FINDINGS: A statement from the director and teacher was received. Per director, it was take your pet to school day. She was the only one who brought her pet to school. She was asked if there was a schedule for the activity and she stated that it was from 10 to 11 but that the dog was in the building all day because she was still at work. The incident happened at around 4:40PM. The teacher grabbed the dog and the dog bit her. She stated that there may have been children in the hallway but doesn't believe that they saw the incident. Per the teacher, she was leaving for the day and there were children lined up in the hallway, against the wall, with the director. The dog was on a leash being held by the director, laying on its back, so she bent down to pet it. While she was petting it, the director asked her to stay with the children while she checked something out in the playground. As the director walked away, she pulled the dog's leash by accident so the dog scratched/bit her near her ear/chin area. She started to bleed and the children got scared. TA was provided on prohibiting animals that pose potential threat of harm to the children in care. This portion of the complaint is substantiated. Some children files were checked for compliance for ages 2, 3, and 4. There is a document on file named "Animal Encounter Authorization" that parents signed upon enrollment and the pet is vaccinated. This part of the complaint is unsubstantiated

Complaint alleges that a child currently in care at Everglades Academy Preschool, referred to be evaluated at KOAKA ABA in Miami and has falsely been assigned a therapist in house at Everglades Academy in Pembroke Pines. The current director for Everglades Academy is in fact an employee of Koala. A complaint inspection was conducted on 2/11/2025 and a statement from the director was received. Per the director, C.B-M., The director stated that Koala is a company that they refer the children to for therapy services. The director was asked if the business is located on-site to which she responded no. They just refer their families to that company for services but don't require that their children use that company. She was asked about working for the school or the therapy company and she stated that she used to be the owner of Koala ABA and that is the reason for her wearing a shirt with the Koala logo on it. Upon arrival to the facility, there are two different decals on the entrance doors. One door decal says Everglades Academy with the phone number and website, and the other door has a decal that says Koala ABA and Learning Centers with the phone number, website and hours of operation being Monday- Friday 7:0AM to 6PM.The director was observed wearing a shirt with the logo Koala ABA and and Learning Centers. I visited Koalaaba.com and under the locations tab, The preschool address is presented as one of the locations for the business and the director of the preschool is listed as the executive director under the about/our team tab. A statement from the owner could not be received as her phone sounded off and my call could not get through. This complaint is substantiated for operating a business unrelated to child care at Everglades Academy

A complaint allegation was received on 1/17/2024 alleging that the van capacity exceeded. Complaint indicated that a van from Everglades Academy had more children on the vehicle then available seat belts. A complaint inspection was conducted on 1/19/2024 and the pickup procedure at Chapel Trail Elementary was observed. The preschool owns a 15 passenger van that has a total of 13 seatbelts available for children. On the date of inspection, only 13 children were picked up from the elementary school. A review of the transportation logs revealed that 14 children were picked up and transported to the preschool on 1/9/024, 1/10/2024, 1/16/2024, 1/17/2024. In the month of December, 14 to 15 children were transported from Chapel Trail to the preschool in a 15 passenger van that only has enough seatbelts for 13 children. According to the Person in charge expressed that there is only one van that's used to transport children. Per the transportation logs, all children are loaded and unloaded at the same time. This complaint is Substantiated for exceeding van capacity. A non-compliance and Notice of Violation is being issued at this time
