LS observed based on statements and via footage, the incident has taken place on Thursday, August 1, 2024, and the provider failed to complete an incident report on the day of. The provider was able to complete an incident report form on Monday 5, 2024. Technical assistance was given to provider per CCF handbook "All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur."
