44-04 · Personnel Records

Per DCF Handbook CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C- 22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. All Personnel records had expired child and abuse forms. All Child abuse and neglect forms should be emailed to the licensing office by 2/5/25

23-02 · Fire Drills

The facilities annual fire inspection expired 08/17/2024. The facility had a fire inspection 08/08/2024 in which corrections were needed prior to an approved fire safety being provided by the local fire authority. The facility will submit a cleared fire safety inspection to the licensing office on or before 09/30/2024 via email

17-06 · Outdoor Play Area/Fencing

There were gaps at the base of the playground fence. As discussed at previous inspection provider will add sand and sod to close gaps. Provider will email picture of completed work to the licensing office on or before 7/25/2024

24-23 · Food Preparation Area

There was no thermometer in the infant fridge. Provider will put thermometer into the infant fridge and email a photo of the thermometer in the fridge to the licensing office on or before 7/25/2024
