32-05 · Outdoor Equipment
During the inspection, the Director was unable to find the monthly monitoring report for the playground. The Director completed the form today during the inspection. The Child Care Facility Handbook states on page 38 that: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. There are currently 70 children enrolled. All 70 children's records were reviewed. One child has been at the facility for over 30 days and has an expired student health exam. The Child Care Facility Handbook states on page 60 that:7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. Two newly hired employees, HP and CB, needed a resubmission of their background screening prior to employment, and it was not completed. The resubmission was completed during the inspection and the Director was informed that the employees were not allowed to be left alone with the children until the updated background screening results have been received. that 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. A re submission should have been completed prior to the employee returning. The Provider will need to do a re-submission on SM. SM cannot be left alone with children until the re-submission clearance is received. The Child Care Facility Handbook states on page 53 that: C.
