Upon on-site inspection, there was still a TV sitting on top of the kitchenette set in the two-four year old room. The spot that the TV was sitting on was narrow and the TV was not anchored to the top of this furniture. [Teacher A & B] reported that the TV was brought in for a lesson plan activity and has not been used before or after that activity. [Teacher A & B] removed the TV from the classroom. On the playground there were two classes combined: one-two year olds (1:5) & three-five year olds (1:10) for a total of 2 teachers and 15 one-five year olds. With these classes combining, the group on the playground was at a one year old ratio (1:6) therefore requiring 3 staff members for this group. Classes came inside and separated, bringing this into compliance. At lunchtime, these two groups combined again. At this point there were three staff members in the classroom; however, two of the staff members were taking turns leaving the room to gather and prep children's lunches. Counselor explained that all three staff members must stay within the classroom to count in ratio. It was suggested to prep all student lunches in the classroom or separate the classes during lunchtime so they only require one teacher each. During on-site inspection, counselor observed 3 one-three year old children eating Dorito chips at lunch time. Teachers put chips back in children's lunchboxes. It is recommended that the director sends a food safety letter home to each parent which includes the below handbook section verbiage. Ultimately, it is staff members responsibility to ensure that possible choking hazards are not fed to children in their care, even if it is sent, from home, in the child's lunch. Per Facility Handbook, Section 3.9.3 Food Safety: C. Foods that are associated with young childrens choking incidents must not be served to children under 4 years of age, such as, but not limited to, whole/round hot dogs, popcorn, chips, pretzel nuggets, whole grapes, nuts, cheese cubes/sticks and any food that is of similar shape and size of the trachea/windpipe.
