Director self reported on 11.5.24. while Z. M-S was in the care of Ms. Luciola she dumped the bleach and water solution out in the sink and placed the empty bottle on the rocking chair, Z. M-S pulled himself up on the rocking chair and touched the empty bleach and water bottle. A follow-up inspection was conducted on 11/8/2024 and a statement was received from the director(W. Sargeant) and teacher(L. Salas). Director's statement: The director was asked if there are cameras at the facility to which she said yes but that they are live feed only. She was asked to confirm that they do not record and she stated that her manager said they do no record. Director, stated that the teacher said the child did not put the bottle in his mouth but the parent says he did. Teacher's statement: The teacher was asked how the incident occurred and she said as follows; It was the end of the day and she had just finished cleaning her classroom. She dumped the bleach and water solution in the sink before attempting to leave the classroom. She had the two empty bleach bottles in her arm when she noticed the window was left open. She put both bottles on the rocking chair and went to close the window and collect her personal belongings. At that time, the parent walked in the door and saw her son standing at the rocking chair holding one of the bottles. The parent asked is that bleach? and she responded yes. The parent was asking why was she cleaning while her son was still in the classroom? The child's hands smelled like bleach so she offered to wash his hands. The parent said no and asked for wipes to wipe his hands instead. After notifying the front office about the incident, she left and came back about 5 minutes later asking for more wipes. Another teacher offered to wash the child's hands and they took the child to the bathroom and washed his hands. Finding: A non-compliance is being issued as staff was cleaning while the room was occupied with at least one child

A complaint was a received that a staff person A. U. was hired at this facility and had left employement at another facility on March 30, 2023. A review of the Background screening and personnel file requirements form (CF-FSP 5 131) indicated that employee (A. U.) last employment was in 2022. The clearinghouse screening shows that she worked from August 2022 to March 30, 2023, at another facility. A phone call was made today, and the assistant director of that facility verified that her last day of employment was March 30, 2023. There was no evidence that the facility conducted an employment history check from August 2022 to March 2023 for A.U
