During a previous inspection TA was giving for ceiling tiles that displayed discoloration, this was repaired. During this visit it was observed that 10 ceiling tiles displayed water stains. In addition, in the infants room one lower wall needs to be touched up with paint and in the afterschool room a wall needs to be painted and a small hole patched up. A carpet in the two year old room needs to cleaned

A complaint allegation was received alleging that On 3/26/2024, after 4pm, staff called mom to make her aware of an incident that involved TW. Reportedly, as staff was sitting in a chair, interacting with some of the other children, she heard a scream and crying. She immediately got up from the chair and went over to TW. Reportedly, he injured himself with a toy. He sustained bruising to the outer area of his eye and scratches. Staff completed an incident report. It is believed that TW did not sustain the injuries from a toy. A complaint inspection was conducted on 3/27/2024 and statements from the assigned teacher (NF) , toddler teacher (AW) and director (TG) was received. Per the assigned teacher (NF), the incident occurred in the afternoon. She had a total of 10 children in the classroom, 5 were playing with her and the other 5 were in another area of the room. She was sitting with one group pretending to be asleep and the children were telling her to wake up and then she heard TW scream. She immediately went to him and saw the scratches on his face. She saw him flapping his hands, rubbing and scratching his face but he already had the scratches when she tended to him. Per toddler teacher (AW), she was in an adjacent room and the classrooms are divided with a half wall and a clear partition. She heard the child scream and looked into the 2y/o classroom. She saw him crying, screaming and scratching his face. She went to him to try to calm him down but she couldn't. When asked if they know how the child sustained the scratches, both teachers do not know for sure. They assume he scratched himself because when they went to tend to him, they saw him scratching. Per Director, she contacted the parents and notified them of the scratches. Incident report given to parents was reviewed and it states that the child has scratches on the left side of his cheek. There are no details to how the child sustained he scratches. The staff denies telling the parents that he scratched himself with a toy. This complaint is Substantiated as the staff is unaware how the child received the scratches to his face. A non-compliance for inadequate supervision is being issued

The provider is using their own authorization for prescription form but did not have one completed for the child. The form was completed in the presence of the monitor. The form was not completed by the staff members who administered the medication. This form must be completed and signed by the staff member who administered the medication each time it is given. TA was provided in that the form should include Medication name, amount to be given, time to be given, dates given, record of the medication name given, parent signature and date. This form must be correct and contain all the components of Broward County No. 5 Form. The record of the medication given does not show the accuracy of the medication provided to the child, the form is missing the record for the month of August 2023
