Vehicle with VIN # 1804 was observed missing the facility's name on each side of the vehicle, and the caution transporting children sign in the back panel. Per provider's statement, the company creating the signs will be installing the signs by next week. Provider was advised to place a temporary sign by the vehicle's windows until the signs are properly installed. Unrelated to the above violation, the vehicle with VIN # 8582 was observed in compliance with transportation requirements. Vehicle insurance for both vehicles is valid until 10/13/25. Vehicle inspection for both vehicles was conducted on 10/16/24

Provider self-reported an incident that occurred on 1/9/2023. During nap time, Ms. AR abruptly flipped a student making the student hit her nose and cry. During release time while the children were watching an educational video, Ms. Alisha grabbed the same child by her arms and sat her down on the carpet. After the teacher sat the child on the carpet, the child touched her arms in a signal of pain. Then while the child was sitting down on the carpet where she was supposed to be, teacher Ms. AR tried to move her over with her feet. During, the same release the same teacher Ms. AR approached the child who was lying on the floor and asked her to sit on the carpet and at the same time pulled the child by the hair and then pulled the child by her arm, lifting her from the floor and then dropped the child on the carpet and the child started to cry while touching her head on the same place where Ms. AR pulled her hair. A complaint inspection was conducted on 1/13/2023 and the video footage of the incident was observed. At around 1:20PM, the teacher was observed abruptly flipping a student making the student cry as mentioned on the complaint. I could not see if and where the child hit herself as the incident occurred behind a cubby. At around 5:06PM, the teacher was observed grabbing the same child by her arms and sitting her down on the carpet. After the teacher sat the child on the carpet, the child touched her arms in a signal of pain but did not cry. The last incident observed occurred at around 5:08PM. The teacher pulled the child's hair and then pulled the child by her arm, lifting her from the floor and placing the child on the carpet and the child started to cry while touching her head on the same place where Ms. A pulled her hair. This complaint is deemed SUBSTANTIATED. The director stated that they do not tolerate such behavior on their school and the children's safety is a top priority. The teacher in question was a new employee and she addressed the incident with the teacher, showed her the footage of the incident and terminated her employment immediately on 1/11/2023. Provider, as a mandated reporter also notified the abuse hotline of what she observed on the footage
