44-07 · Technical assistance was provided regarding this standard
ALLEGATION: A five-year-old child was left at the elementary school where she was supposed to be picked up by Lil' Rascals Academy' transportation service. Parent was concerned about the child's safety since the child alleged that she was placed in a different bus and mom was not informed. INVESTIGATION: An onsite investigation was conducted on 10/01/2025 which consisted of interviews with the facility's administrators and staff member in charge of picking up the child. Upon arriving to the facility one of the schools' administrators NP was informed about the incident and stated that the director S.A. was the person that had more information regarding this incident and provided a copy of the transportation log and attendance recorded on 9/26/2025. School director was not on site at the time of the inspection but contacted CCLE via phone. On this conversation, the school director S.A. stated that on 9/26/2025 she noticed one child did not arrived at the center and proceeded to call the elementary school to verify why the child was not ready for pick up. The elementary school did not answer her phone call but returned the call to inform them that the child was left there. Program's director S.A. accessed another approved vehicle and proceeded to pick up the child. According to S.A. the school did not provide clear information of the child's whereabouts, only that the child might have been in the bathroom at the time of dismissal. Both school director S.A. and school staff member K.M in charge of picking up children stated that the standard procedures for pick up are the following: • The elementary school placed all children in a group (green group) prior loading the bus • Once children are loading the vehicle, the facility staff member starts checking in children on the transportation log • The facility's staff member only loads all children placed in line by the elementary personnel. • Elementary staff member confirms with a thump up that all children that are supposed to go in this vehicle is complete. • Children that do not load the vehicle are marked absent on the transportation log. Based on the information gathered, the allegation is deemed UNSUBSTANTIATED, however this standard is marked as non-compliance since the program failed to follow procedures by not having second childcare personnel signing to verify that all children were counted and that the log is complete. In this case, the driver signed both areas of verification

A complaint was received on 1/11/2024 alleging that the child sustained an injury and by the time the child was picked up at 5:30 PM, her face was swollen. Staff didn't give the mother any explanation. The director stated that it was an accident. The teacher put ice on the child's face and allowed her to sleep. No one contacted the parents when the incident occurred at 10 AM. A complaint inspection was conducted on 1/12/2024 and a statement from the teacher(DD) was received. Per the teacher, the child was running on the playground when a boy opened a gate that led to the outdoor snack area and ran in. The little girl (JR) ran after him, fell and hit her face on the picnic table. The teacher was running behind them but she didn't get to her in time before she fell. The teacher notified the office of the incident immediately after occurrence. The office would write the incident report but it was not provided to the parents at pickup. The staff applied ice and the swelling had gone down. After the incident, the child had a normal day. An incident report was checked for compliance. Per the incident report, a child crashed into JR causing her to hit herself on the table. This complaint is Substantiated as the parents were not provided an written incident report on the date of occurrence. Immediate notification of incident is Unsubstantiated as it is not required per minimum licensing standards

A complaint allegation was received on 1/26/2023 alleging that Mother went to pick up her 2y/o daughter in aftercare and saw a small boy (LM) between 2-3 years old alone in the classroom with no supervision. The mother took his hand a walked him to the front desk office and reported to teacher no supervision was there . All other kids were outside. A complaint inspection was conducted on 1/30/2023 and a statement was received from the assistant director(CR). The director (SD)was not on-site during today's inspection. The area supervisor(KB) was present and witnessed today's interview with the assistant director. Cameras were observed in the school but they do not record. Per the assistant director, the cameras are only live feed. Per statement received from CR, the incident occurred during transition period on 1/26/2023. The 2y/o classroom was transitioning out of the classroom and into the playground to have snack at about 2:45 or 2:50PM. A mom arrived shortly after, at around 2:57PM to pick up her daughter from the 2y/o class and that is when she found the little boy in the classroom, playing in a play area, near the entrance door. The mom then brought the child to the front office and informed the office staff that the child was found alone in the 2y/o classroom. Per the classroom attendance, there were 14 children present on the date of the incident. Assistant director stated that there were two staff in the classroom, an assistant teacher (S) and the lead teacher (R). A statement from the teachers couldn't be received as the teachers are no longer employed at the child care center. The center has terminated the two staff members due to the lack of supervision and because a transitional training was provided to the teachers about 2 days prior to the incident. This complaint is Substantiated and a notice of violation is being issued at this time
