The following ratios were observed; During initial walkthrough of the school, I entered the infant room and observed one teacher on the floor with some infants (total infants in room: 7 ) and the second teacher walking out of the room. I stood outside the room, near the new white barrier that was added in the room and one infant started climbing on top of the other a few seconds after the teacher left the room. The teacher in the room rushed to aid them. Shortly, the teacher returned and I asked her where did she go and she responded that she went to put the mop bucket away. The teacher was reminded that she needed to remain in her classroom to maintain ratio. Infants 2:7 1y/o 2:10 2y/o 2:10 3y/o 1:12 VPK 1:13 VPK 1:5

Complaint alleges that the daycare consistently appeared to be understaffed, often relying on the director to move between" rooms. Breakfast was held in one classroom with mixed age groups, supervised by a single staff member. Most alarmingly, a 2-year-old child was found alone in an empty classroom during breakfast hours. FINDINGS: A complaint inspection was conducted on 6/13/2025 and a statement from the owner was received. Per the owner, with regards to the allegation with the child found in a room alone, the teacher was taking the 2y/o children to the 2s classroom from the breakfast room. The teacher was leading about 3 children into the room when he went the wrong way toward the 1y/o room and the teacher redirected him. The parent that observed that was very upset but the child was not alone in a room. During today's inspection, a walkthrough of the classrooms was conducted and the following was observed; At 7:40AM, there were only 3 children of mixed ages, including an infant, with 2 staff members. At that time, the classroom split up and the director remained in a room with two 1y/o + children and the second teacher took the one infant to the infant classroom. By 8:03AM, there were 10 children of mixed ages (1 to 4) in a room with one staff member. The class remained out of ratio from 8:03AM until 8:13AM when the second teacher entered the room. At that time, the second teacher took the 1y/o children to another room

Complaint alleges that a 15-month-old child was sat in an infant bouncer, claiming it was to prevent him from leaving the classroom if the door opened. This is highly inappropriate, especially given that infant bouncers are intended for babies no older than 6 months per safety guidelines. FINDINGS: A complaint inspection was conducted on 6/13/2025 and a statement was received from the school owner. Per the owner, the teacher had to change another child's diaper so she placed him on the bouncy seat. She did it to prevent him from running out of the classroom when a parent walks in. The child was not in the bouncy seat for more than 5 minutes and it was not to restrain him. The bouncy seat is an infant seat (baby joy) designed for infants ages 0 to 6 months with a weight limit of 20 lbs. Provider was informed that Bouncy seats are designed for infant use and not for children 1 year of age or older. This complaint is substantiated
