During the inspection, a third staff member came into the Toddler Classroom, making the ratio 3 teachers to 14 one year olds and two year olds. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group

The director advised that there is one driver. Driver 1s license will expire on 1/15/2027, the physical was conducted on 3/24/2025, and the driver has completed the transportation training. 6-1: During the inspection, Licensing Specialist observed that the 1 driver did not have CPR-Frist Aid training. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.1 The driver of any vehicle used by a child care program to provide transportation must have the following: A. A valid Florida drivers license including the proper endorsement; B. An annual physical examination which grants medical approval to drive, and valid certificate(s) of course completion for first aid training and pediatric cardiopulmonary resuscitation (CPR) procedures

During the inspection, Licensing Specialist reviewed the background Screening Clearinghouse Roster. Care Provider Background Screening Clearinghouse: The Background Screening Clearinghouse will generate e-mails and send you, the provider, a reminder at 60, 30, and 21 days prior to the retention date expiration. Please note, you will only receive that notification for the staff that are on your facility/home roster. Please keep your roster current in the Clearinghouse to avoid confusion on expiration dates, or unwanted notifications. 45-9: During the inspection, Licensing Specialist observed that the background screening employee/contractor roster was not maintained in that there were 12 employees that were added within 10 days of their hire date. During the inspection, the director added the staff to the roster. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance CCF Handbook, Section 5.1, K The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a child care eligible result and has been hired at the facility. During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there are 15 employees. During the time of the inspection, the Licensing Specialist monitored 15 of the 15 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. 33-1: During the inspection, Licensing Specialist observed 1 child care personnel did not complete the DCF 40 hours of introductory training within 12 months of starting the DCF courses, exceeding 15 months. During the inspection, Licensing Specialist provided technical assistance, During the inspection, Child Care Regulation provided technical assistance, Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. 39-5: During the previous inspection, Licensing Specialist observed that there were 7 accident/incident reports that did not have the time of the accident/incident on the report or, 11 reports did not have the childs last name, and 3 reports did not have the date of the incident/accident. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.4, C Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. 07-06 · Vehicle Requirements
During the inspection, Licensing Specialist observed that the program's vehicle was monitored at time of inspection. The installed vehicle alarm was not in working condition in that it could not be heard from the distance of 500 feet from the vehicle. The alarm did not sound it just made a clicking noise. The director was given until July 8, 2024, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, regarding the maintaining of the vehicle alarm. CCF Handbook, Section 2.5.4, F - F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare. 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above (p. 17)

During the inspection, Licensing Specialist observed holes in the wall by the sink in the 3 year old room. The director was given until July 8, 2024, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months
