• Initial ratio • 7 Months - 1 year old (1:4) 1 staff • 1 - 2 Year (1:6) 14 children 1 staff • 3 - 4 Years (1:15) 16 children 1 staff (Naptime Ratio) FSC observed staff for 2-year-old classroom not in the building during nap time and staff for 1-year olds was in hall between the rooms overlooking both 1 year old group of 4 children and 2-year-old group of 10 children, fixed at time of inspection by having staff return to room. Resulting in the following ratio: • 7 Months - 1 year old (1:4) 1 staff • 1 Year (1:6) 4 children 1 staff • 2 Years (1:11) 7 children 1 staff • 3 Years (1:15) 9 children 1 staff • 4 Years (1:20) 7 children 1 staff T.A. The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. 2.4.2 Nap time Supervision During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. Rooms did not have sufficient lighting LED light meter was used with a result of 1.1 Lux, Provider was made aware and fixed at time of inspection by moving some curtains to allow some light to come in. T.A. During naptime, lighting must allow child care personnel to visually observe and supervise children. Lighting must be sufficient to allow for adequate supervision and safe entering and exiting of the room. All areas of the facility must have lighting that provides adequate illumination and comfort for facility activities, a minimum of 20 foot- candles of lighting is required

Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.
