39-07 · Accident/ Incident Notification and Documentation

When counselor arrived at facility to conduct a routine inspection today [Director] reported an incident that occurred on 10/25/22. [Director] stated that [Child A] fell when getting off a piece of playground equipment, landing on her right arm. Parent picked up immediately and took child to emergency room. Parent contacted facility later that day to inform them that child broke her arm. At the time of the incident, [Director] notified partnering agencies and an incident/accident report was filled out and signed by parent, but it was not immediately reported to licensing authority as required by Child Care Facility Handbook, Section 6.4, E
