With three accident/incident reports being written for the three children involved in the accident/incident, none were written on the day of the accident/incident (11/26/2024). They were only written the following week (12/2/2024) upon request from one of the parents. They were dated on the date of the incident even though they were not written at that time. Childcare Networks Behavior and Positive Guidance Policy states that an Individual Action Plan will be implemented for repeated behavior concerns and that, if the Individual Action Plan is not successful, the school may determined appropriate response to separate the child from the school on a temporary or permanent basis. The policy also states that examples of behavior concerns include excessive biting. [Child B] has been biting consistently since 7/2023 (appx. 56 incident reports for biting) and, at time of on-site inspection, no Individual action plan has been put into place and no temporary or permanent separations have happened. On 3/7/24, [Child B]'s parents signed Individual Action Plan, it went into effect immediately, and District Manager emailed copy of it to counselor

A school age child was left on the bus unattended. The child got off the bus through the emergency door, knocked on the childcare facility door, and childcare staff let her into the building. The Provider will complete a transportation and supervision training for all childcare personnel who assist with transportation services. Upon entering the younger toddler classroom counselor observed one teacher with 7 one year old children. Teacher A stated it was water day and there were 4 children still outside with a Teacher B. Teacher A sent one child back outside with Teacher B, bringing her ratio inside back to 1:6. Upon entering the two year old classroom counselor observed Teacher C with 14 two year olds. Shortly after, Teacher D walked into the classroom from the playground. Teacher D stated that the children had brought outside toys in with them and she was putting them back outside. 04-04 · Supervision
[Employee C] noticed [Child A] sitting around a corner outside while her class was being called inside. Before [Employee C] went inside she told [Employee B] that the child was sitting there. [Employee B] brought her class inside, leaving [Child A] on the playground. Within 5 minutes, [Employee C] noticed [Child A] outside the window of [Employee B]'s classroom. [Teacher C] ran across the hall, into the child's classroom and brought the child inside. [Employee B] was counseled by management regarding supervision. Management also held a mandatory staff meeting on 4/22/23 and reviewed proper classroom supervision/interactions, proper playground supervision, playground transitions and proper attendance taking. [Employee B] also told counselor that she regularly stands in the doorway to allow children to use the bathroom while the rest of the class is outside. Counselor advised that this is not proper supervision in childcare and if children need to use the bathroom while class is outside she should either call for assistance or have child go into another classroom (with supervision, in ratio) to use their bathroom

23-02 · Fire Drills
Previous fire inspection expired on 3/16/23. Fire panel box was in silent mode due to issues are stemming from an HVAC issue. Assistant director was afraid if they took it out of silent mode to do a test (drill) she would not be able to turn it off after. HVAC company, monitoring company and fire marshal were on-site 4/5/23 to fix these issues. All issues were fixed and a fire drill was observed at follow up re-inspection on 4/7/23. Updated fire inspection passed on 4/5/23. Fire drill log was retained for licensing file. Fire extinguishers were tagged 2/2023
