Facility Handbook, Section 3.6.1 Bedding and Linens Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. G. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. On Friday March 8th, 2024, [Child A] was left in a classroom unsupervised for 10 minutes. [Teacher A] and [Teacher B] transitioned out of the toddler 2 classroom to go across the hall and outside to the playground at 2:33 PM. A teacher in the classroom next door heard the child crying and notified management. [Management] opened the door at 2:43 and found [Child A] in the classroom unsupervised. Management returned [Child A] to her teachers outside

According to [Management] and [Teachers A & B], there was no incident report filled out regarding this situation and, as of counselor's on- site visit on 3/12/24, parents still had not been notified of the incident. [Management] was advised to call parents immediately to inform them of the incident. Accident/Incident report should be filled out, signed by parents, and emailed to counselor no later than 3/29/24

At 2/27/23 routine inspection, facility received a violation for [Teacher A] working while she was not in compliance with required DCF training. On 3/24/23, violation was cleared when Director reported that this employee would not be on-site until training was passed. Employee was working on 6/9/23, when counselor was on-site for complaint inspection/interviews, and DCF training has not been completed. Employee should complete all required DCF training and Director should email counselor updated transcript no later than 6/30/23
