Teacher failed to document an accident on the day that it occurred. While she verbally informed the assistant director at the end of her shift, neither she nor the assistant director wrote an accident report. When the director heard about the incident the next morning, she had the teacher write an accident report and shared it with the parent, who signed the report leaving the following comment: "Was not notified about this incident until following day. This incident was prior to another that happened later that day. Why was I not notified about this one but was about the other?"

All toileting facilities need to have paper towels available at all times. Since the facility ran out of refills for the paper towel dispensers, provider needed to supply alternate paper towels. Cloth towels used in the meantime are not permissible. Facility Handbook, 3.7 Restrooms and Bath Facilities H.Running water, soap, trash receptacles, toilet paper, and disposable towels or hand-drying machines that are properly installed and maintained must be available and within reach of children using the bathroom. Provider will purchase paper towels and supply each handwashing sink with a paper towel roll or napkins until refills for the dispensers are received. Provider will send a picture of paper towels provided and/or of filled dispensers to the licensing office on or before 2/6/2025

Teachers were preparing food items brought from home such as macaroni and cheese bowls in the classroom without an appropriate food prep area. Items brought from home can be heated up in the classroom microwave but mixing and combining ingredients is considered food prep.
