Allegation: Complaint alleges that the preschool is understaffed. There are times that a staff member will be responsible for 30 children at a time and take them outside. At times those staff members will leave the children outside, stepping inside for a minute or two, leaving the children unsupervised. If a teacher has to use the restroom, the children are left unsupervised. FINDINGS: During onsite investigation on 4/30/2025, A lack of direct supervision occurred on two different instances. Upon arrival, Teacher A was observed walking into the classroom with four 1y/o children. The teacher led the children into the 1y/o classroom and proceeded to open the door for me, leaving the children unattended in the room. Then, teacher walked down the hallway toward the VPK room. I stopped her and asked her if she is an adult required for ratio in her class and she said yes. I told her to go back into her class and I would go to the VPK room to speak to the VPK teacher. A walkthrough of the classrooms was conducted and staff communicated to me that some of the children were in swim class at the swim school located next door. At around 11:30AM, 10 children from ages 2 to 4 were observed walked back to the school with 3 adults. One of the three adults was teacher B, an employee of the preschool. The other two adults were employees of the swim school. After getting further information from Teacher B, it was determined that the children are being picked up by swimming school staff at around 9:49am, signed out in a log sheet and walked to the swim school. Teacher B stated that she walks with them to the swim school to change the children's clothes and then walks back to the school leaving the children with the swimming instructors for the duration of the lesson (1 hour - 10:00AM to 11:00AM). At 11:00AM, she walks to the swim school, changes the children's clothes, and walks them back to the school. She stated that swim lessons are held on Wednesdays for the duration of one hour. Complaint is substantiated for a lack of supervision. A class 1 lack of direct supervision and notice of violation is being issued

Upon arrival, the director was found at the desk and the toddler teacher was found in the office on break. Once having entered the toddler room to initiate the inspection, it was discovered that because of this, the classroom of six one year olds had been left unsupervised. The teacher did enter the classroom before the inspection started. When the director was questioned if the teacher on break was indeed the toddler teacher and thus the classroom had been left unsupervised, she indicated yes.
