Complaint was received on September 21, 2023 and investigated on September 22, 2023. Complaint states the following: There are concerns for child being injured at the childcare. The teacher was taking out the garbage and he was crawling after her. The door was propped open, she did not see him, and his finger was wedged in the door. He lost the tip of his finger. He was taken to the hospital for treatment An incident report was reviewed and the teacher was interviewed. Each indicated that the teacher had opened the door to place the garbage right outside of it. When she turned to close the door, she did not see that the child had crawled toward her. The school has door guards on all of their classroom doors however, the child's finger was just below the guard so the door was closed onto his finger. Based on the results of this investigation, this complaint is substantiated
