LS observed Soiled or wet diapers, clothing, or linens were not changed immediately. A child was left with the same diaper for the whole day. Per the provider the teachers have been spoken to about the incident. 04-04 · Technical assistance was given to provider per CCF handbook "Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised."
At the date of incident children were inadequately supervised during outdoor activity resulting in a harm of a child, it that a child was able to pull down undergarment and was poked by the rectum with a small stick beside the slide being by 2 other classmates. Other children were able to watch while in the care of multiple child care personnel. Provider was able to correct at the time of inspection by acknowledging incident and preventing it from continuing, along with taking the "Supervision" training with the licensing authority

12-04 · Facility Environment
FSC observed areas used by facility for after care activities to have possible strangulation hazard with dangling wires hanging and exposed nails present. Provider was able to correct at the time of inspection by removing items from the reach of children. Technical assistance was given to provider " A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. This includes grease build-up in ovens, stoves and food equipment. C. Animals must be properly vaccinated (if a vaccination is available for the specific animal), free from disease, and clean. Animals that are poisonous, venomous, aggressive, or pose a potential threat of harm to children in care are prohibited. Parents/guardians must be informed in writing of all animals on the premises. Notice may be provided by a conspicuously posted notice or bulletin, policy handbook, parent flier, or a statement included on the enrollment form. Current vaccinations records must be available for review by the licensing authority. No animal may freely roam the indoor/outdoor premises. Animals are prohibited in areas where food is prepared, stored, or served. If animals or birds are kept in classrooms, they shall be caged and cages must be kept clean. D. Strangulation hazards, strings, and cords long enough to encircle a childs neck such as those on toys and window coverings must not be accessible to children in care...."

This provision is not affected by a change of ownership as long as the program remains continuously licensed at the original site. Technical assistance was given to the provider per CCF handbook " Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. "
