A call was received by this monitor on February 23, 2026 and investigated on the 24th claiming that this facility was utilizing a former staff members transcripts for a current employee because she did not meet the training completion deadline. Upon investigation, the facility claimed that the current employee had been suspended until training completion. However, during the investigation, this monitor was made aware by the original claimant that the current employee had left out the back door. Her transcripts were then observed and she was outside of her training deadline. The cameras were viewed and the employee was observed leaving the building out of the back door. This portion of the complaint is substantiated for training completion exceeding 15 months
