Complaint received by this inspector on 1/21/25. It was investigated on 1/22/25. It stated the following: The parent of MR got a phone call from the school stating that the child fell on the playground. The complaint further alleges that there was blood pouring from 3 front teeth, upper lip was busted and teeth were broken. The staff present during this incident was interviewed. She indicated that she was sitting at a picnic bench on the playground facing the basketball court. She further stated that she did not see the incident occur as she was talking to a child who was standing in front of her. MR approached the staff member. Another child witnessed the incident and told the staff member what happened. Based on interview and the incident report written by staff and signed by the parent, ice was applied and the area cleaned with water. This portion of the complaint is substantiated as the teacher did not position herself in a manner that allowed for adequate supervision

Upon entering the infant room to conduct the re-licensing inspection it was noted that there were four infants present but only three cribs with sheets on the mattress. The sheets on the three mattresses were observed to not fit tightly and need to be replaced with sheets that fit tightly as required. The teacher in the room was asked what happened to the fourth crib sheet she stated at first that an infant had wet the sheet and that the sheet was in a bag to go home to be washed. When reminded that infants have to sleep in cribs/play yards she than stated that two infants had wet their sheets and that one of the three sheets noticed on the other side of the room had been placed there as an extra. It is recommend that the facility maintain extra tight-fitted sheets on site to be used in case an infant wets the sheet they bring from home and it has to be removed
