During a complaint investigation on May 29, 2024 it was determined that incident reports were not being written on the day that the incident occurred and that the parent or individual authorized to pick up the children did not sign the report. A revisit was conducted today and it was determined that although incident reports were written on the day of the incident many of the reports were not signed and/or dated by the parent guardian or the individual authorized to pick up the child

During the course of a complaint inspection regarding an emergency incident that occurred in the infant classroom, a lack of direct supervision was observed on video surveillance; The incident occurred at 10:03AM. The teacher (IE)was sitting on the floor with the babies. The child was observed on the floor, almost leaning on the teacher's leg(IE). She touched MC and appeared to be checking him, she then picked him up, and two other teachers are seen speed walking from the toddler room to the infant room. Ms. IE rushed out of the infant play area where she is met by Ms.DS near the diapering table. Ms. DS is seen grabbing the child and rushing to the hallway as Ms. IE followed behind. Ms. IE and Ms. DS are seen entering the hallway and the second toddler teacher returned to her room. The infant classroom was left unsupervised as Ms. IE went to the hallway following Ms. DS and the child. The section number cited in the inspection report is incorrect. The facility is being issued a Notice of Violation for violating Section 7-33 (e),Ordinance

During the course of a complaint inspection regarding an emergency incident that occurred in the infant classroom, the following non- compliance was confirmed via attendance/ staff statement; During an incident that occurred on 5/14/2024, the infant classroom had five infants with only one teacher(IE). The second teacher (MA) was in the lobby receiving a baby that had just arrived during the incident causing an out of ratio non-compliance in the classroom. Copy of attendance received. TA was provided that ratios must be maintained at all times, including during morning drop-off times

A complaint was received on 5/20/2024 alleging that while MC (4/6/23) was at preschool, he was observed to be choking on something by his teacher, and the teacher grabbed him and tried to perform CPR, but it is believed by the medical professionals that she did not know how to properly perform CPR. Per video surveillance, First Aid is what was performed. Per the Fire Rescue's report, the teachers stated that they gave the child multiple back blows until they dislodged what the child was choking on. Fire Rescue's report stated that the child seemed to be lethargic with a poor breathing pattern. Staff First Aid /CPR was checked for compliance. All the teachers have First Aid/CPR training but per Director, they were all taken via Zoom. The company used for all but two staff is JV Brothers Consulting Corp. TA was provided that on-line CPR courses are not acceptable to meet this standard

A complaint was received on 5/20/2024 alleging that While MC (4/6/23) was at preschool, he was observed to be choking on something by his teacher, and the teacher grabbed him and tried to perform CPR, but it is believed by the medical professionals that she did not know how to properly perform CPR. MC was beginning to turn blue and was rushed to the emergency room. The medical staff had to properly perform CPR on MC. The incident report was not given to the parent until 2 days later after his request. A complaint inspection was conducted and a written statements (written on incident reports) were received from all staff dated 5/14/2024. Parent was contacted and a statement was also received. Parent stated that after returning home form the hospital, he returned to the school to speak to the Director on the date the incident occurred. The Director showed him an incident report that documented that the child turned blue and CPR was performed. When he requested a copy, the Director said she would email it to him. The parent did not receive the email until 5/16/2024. On 5/16/2024, a different report was sent to the parent and it no longer stated that child turned blue and that CPR was performed. This complaint is Substantiated as the parent was not given a copy of the incident report on the date of occurrence. Written reports received form the teachers were not the report that was given to the parent
