    A child in the one-year-old group was left outside on the playground. This went unnoticed by the teachers for approximately 4 minutes. A staff member from another classroom saw the child and was able to keep eyes on the child and call for assistance. Corrective action has been taken. The Director scheduled a staff meeting immediately to re-train staff regarding the face to name attendance sheet which is designed to prevent this from happening.
