All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. There were several stained ceiling tiles observed by the Licensing Specialist (LS) during this onsite inspection: in the Purple, Green, Red, Blue and Orange room. Pictures were taken of these tiles. LS gave technical assistance to the director to please check for water leaks and have the stained tiles replaced by the due date

Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. In several classrooms sippy cups and water bottles brought from home were not labeled or only had a first name (Pictures were taken). The staff person and director were educated by the licensing specialist on the requirement for last and first names on cups and bottles brought from home. The staff person then correctly labeled the items, thus this regulation is marked back into compliance

12-02 · Facility Environment
At the time of the inspection, the counselor observed areas in the Blue Classroom that needed repair. The tile over the single handwashing sink was broken and the drywall was exposed. The tile was removed at the bottom of the double sink area and there was some pealing drywall, smail holes, and larger hole on the left side. The section also needs to be repainted where the drywall is exposed. A sink cabinet had a piece of loose plywood covering the cabinet with a wood handle protruding. Photos taken. Explained the staff the areas needed to be repaired. Provided technical assistance from the handbook 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

At the time of the inspection there were metal fence posted stacked in a corner of the older children's playground. The ends of the metal fence posts had exposed sharp edges. 41-02 · Immunization Records
At the time of the inspection, the counselor observed two child's files had an expired immunization record. Provided technical assistance from the handbook Section 7.1, Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. At the time of the inspection, the counselor observed three child's files did not have a current Student Health Examination. Provided technical assistance from the handbook. Section 7.2, 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program
