The facility has surveillance cameras in various parts of the facility including the two-year-old classroom. The Counselor observed the video footage of the incident which took place on 10/16/2023, at approximately 10:30 a.m. Counselor saw 5-year-old A.J. in the classroom with the two-year-old group. The 5-year-old was running around the classroom and opened a door leading to the outside and left the facility grounds without the teachers knowledge. The child crossed the highway and ran down the highway away from the facility. The two- year-old teacher was not adequately supervising A.J. and did not see the child open the door and leave the facility. A known civilian saw the child on the highway and brought the child back to the facility. Five-year-old A.J. did not sustain any injuries
