During the time of inspection, Director advised Licensing Specialist that a child was bitten four times by another child. The Director advised that they were only aware of two bites on separate occasions. The Director advised that the parent later told them that the child was bitten two additional times. The Director advised the Licensing Specialist that parent disenrolled the child. This standard was brought back into compliance during the time of inspection. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. 07-06 · Vehicle Requirements
During the time of inspection, the Licensing Specialist observed the alarm on the yellow bus (vin# 94480) did not work. CCF Handbook, section 2.5.4 Vehicle Requirements F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. During the time of inspection, the Licensing Specialist observed a large section of paint peeling off of the wall by the cubbies in the school age classroom. Licensing Specialist observed the air vent to be slightly dislodged from a bulging section of the tile ceiling in the school age classroom. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Licensing Specialist observed weeds in multiple locations on the playground that could pose as trip hazards, two nails sticking out of the fence bottom, litter, an open plastic blue and white storage bin, and a green hose, inflated teal and blue pool, empty plastic cup, paper towel and leaves were observed inside the container. A bin cover was observed on the ground by the metal bench, the bin cover had two broken corners. Director informed Licensing Specialist that she had Scheduled a lawn service that will take place during this week. 17-06 · Outdoor Play Area/Fencing
Licensing Specialist two broken fence panels, one by the entrance that leads to the VPK class the gap is open to the facility parking lot the gap was measuring 4 inches, and the second was located on the back side fence of the large playground, the gap is open to a shopping plaza parking lot, the gap was measuring 5 3/4 inches. CCF Handbook, Section 3.f, F. 3.5 Outdoor Play Area F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build-up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. Licensing specialist observed two rusted areas on the large playground permanent equipment. One was located on the metal board that was connected to the yellow metal ladder (the board was covered with black material that was covering the metal base. The base was worn from the corners exposing the rusted metal board underneath). The second one was located on the back of the black climber wall leading up to the red top connected to the large red slide. 46-01 · Daily Attendance
Licensing Specialist observed that the teacher responsible for the school age class did not have the daily attendance sheet available. Licensing Specialist asked the teacher of how many children did she have in the class she said 18, Licensing Specialist counted 19 children in the class. Assistant Director brought the class attendance sheet from the front office. 19 children were counted on the attendance sheet and in class, thus returning this standard into compliance at the time of this inspection.
