12-02 · Facility Environment
During the time of inspection, Licensing Specialist observed several ceiling tiles needing replaced that had holes and cracks and water damage in all of the classrooms. The paint on the walls were peeling or chipped in all of the classrooms. The lights were flickering and need replaced in the three-year-old and VPK classrooms. There was a hole in the window seal of the first window as well as damage to the wall under the window in the three-year-old classroom. The doorknob on the door leading to the bathroom was loose in the three-year-old classroom. There was a hole in the wall under left window and rust around handwash sink in the VPK classroom (#7). CCF Handbook, section 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children

During time of inspection, Licensing Specialist observed outlet covers missing in the two-year-old, three-year-old and VPK classrooms. 17-01 · Outdoor Play Area/Fencing
During the time of inspection, Licensing Specialist observed on the 3's and VPK playground: • The A/C vent screens to be rusted with sharp edges on the 3's and VPK playground. • Two brown doors on the exterior of the facility leading to the 3's and VPK playground have peeling/chipped paint. • There are holes along white panel fence going around the perimeter of the playground. • Sharp edges around outdoor window on the 3's and VPK playground. • There are holes in the unitary surface under play equipment and around running/walking track that could cause a tripping hazard. • Water draining from outdoor pipe near picnic tables. During the time of inspection, Licensing Specialist observed on the Back Playground Lot: • Several cut trees/debris lying in a pile. • White broken fence panel along top right-hand side and broken panel on lower right-hand side. • Gap within white panel fence which could cause fingers to be pinched. • White fence post lying on ground which could cause tripping hazard. • Square concrete block on ground. • Rusted lock/hardware on white gate leading to parking lot. 18-12 · Bedding and Linens
During the time of inspection, Licensing Specialist observed the bedding, and linens were stacked together in closets and drawers in the two-year-old and both of the three-year-old classrooms. CCF Handbook, section 3.6.1 Bedding and Linens Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. G. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. 44-01 · Personnel Records
During the time of inspection, Licensing Specialist observed that 3 new hires were missing required documents from their files such as Background Screening, Child Abuse and Neglect, 5131, Good Moral Character and Employment Attestation. CCF Handbook, section 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. D. Copies of training information and credentials as applicable. 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B. CF Form 1649A, Child Care Attestation of Good Moral Character, which is incorporated by reference in 65C-22.001(7)(a), F.A.C., must be completed for all child care personnel at the time of initial screening or upon change in employers. CF Form 1649A may be obtained from the Departments website at www.myflfamilies.com/childcare. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. D. A copy of the DCF letter/email informing of search conducted of the Floridas child abuse and neglect registry must be on record for each personnel screened between July 1st and December 15th of 2016. E. A copy of each request made to out of state child abuse and neglect registries for individuals who lived outside the state of Florida in the preceding five years. F.
