05-04 · Transportation
5-4: During the inspection, Licensing Specialist observed that some transportation logs did not include documentation that the children got off the vehicle at the destination and documentation that the children got back on the vehicle after leaving the destination, the secondary signature or the driver signature. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.2 A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 6 employees. During the time of the inspection, the Licensing Specialist monitored 6 of the 6 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. 33-6: During the inspection, Licensing Specialist observed 1 employee did not complete the DCF 5 hour early literacy training within 12 months of their child care industry start date. During the inspection licensing specialist observed that the first restroom light was down to one fluorescent light bulb and it was flickering. Be sure to have the lights replaced in the restroom by the due date. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. 35-12 · Communicable Disease Control
During the inspection licensing specialist observed that the two new staff members did not have exposure plan training documentation. Be sure to have staff trained on exposure plan by due date. • During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6 Health Requirements There are three common modes of transmission for the spread of microorganisms in child care settings: contact, droplet, and airborne. Many common infections encountered in the child care setting are transmitted by direct or indirect contact. Child care facilities shall develop a written exposure plan regarding universal safety precautions, recommended by the Centers for Disease Control and Prevention (CDC), to follow in the event there is exposure to blood and potentially infectious fluids. Personnel are required to be educated on the facilitys exposure plan regarding standard precautions before beginning to work and annually thereafter. At time of inspection, licensing counselor observed and photographed snacks in the school age refrigerator stored improperly. The vanilla wafers open box was in a bowl with the box top open and the foil bag just folded down not clipped closed. No date was on the box as to when the wafers were opened.
