During the inspection, Licensing Specialist observed that the transportation log was missing the 10 second childcare personnel signatures in the month of November for the following dates: 11/4/24 in the PM, 11/8/24 both AM/PM, 11/14/24 in the AM, 11/15/24 in the PM, 11/19/24 in the PM, 11/20/24 both AM/PM and 11/21/24 both AM/PM. The director was given until January 2, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook section 2.5.2 Transportation Log -A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the childcare facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person (s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

During the inspection, Licensing Specialist observed on the small playground the bottom hinge to the white gate that is connected to the second building is broken leaving the gate open(the area that it leads to has pipes, boxes, dolly but it leads to another fence as well. 32-07 · Outdoor Equipment
During the inspection, Licensing Specialist observed that the mulch around the yellow bus and the orange, green, red and blue bouncer did not meet the 6-inch depth requirement, in that the depth of mulch was 2 inches in depth. 03-01 · Ratio Sufficient
At the time of reinspection, licensing specialist entered the front building that had 5 sleeping 2 year old children and no staff members were in the building. 04-02 · Supervision
At 1:00pm licensing specialist arrived at the facility and the sign on the front door stated to use the side gate entrance. Licensing specialist entered the front building via the side entrance of the facility to find 5 children asleep on cots and mats. No adult was in the building. At 1:30pm the staff member returned to the building. Licensing specialist asked staff if she was just entering the building now and staff answered, "Ya I had to step right outside to talk to the neighbor over there". A prospective family was trying to enroll their child and followed the staff member back into the building. The prospective family were leaving when there was no answer at the door and exited the fence to go to their car when the staff member came across the driveway from the side closest to First St. -to the right of the building if facing the front of the building. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location

In a previous inspection licensing specialist observed and photographed diaper changing procedures not posted at diaper changing area. At the time of reinspection, no diaper changing procedure was posted. Licensing specialist emailed the provider a copy of the CDC's recommended diaper changing procedures in a childcare facility and director was able to post the diapering procedures in the diapering area. • During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.10.2, L Diapering Requirements- L. During the reinspection, licensing specialist observed the mulch to be only 3" under the stationary play equipment. Be sure to add mulch by September 28, 2024. Director was able to locate a lid and covered the open trash can bringing this back into compliance. During the inspection licensing specialist observed and photographed cleaning supplies (Clorox Bleach and several bottles of Odo Ban) in the corner of the open kitchen area along with a mop bucket with water. The staff member was able to move the cleaning supplies to under the sink where a child lock was on the cabinet bringing this back into compliance, During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.2 Toxic/Hazardous Material/Firearms/Weapons B B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. 23-07 · Fire Drills
During the inspection licensing specialist observed that the fire drill log did not have a date for the August fire drill. Due to lightening and thunder in the area a fire drill could not be conducted. Be sure to have a fire drill by August 30, 2024 During the inspection, Licensing Specialist provided technical assistance, CCF Handbook 3.8.4 A-C Fire Drills. During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. Child Care Facility Handbook Page | 28 B. When the facilitys approved fire alarm alert system is activated or initiated, all adults and children must evacuate the facility. C. A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. 07-06 · Vehicle Requirements
Vehicle alarm installed not in working condition 2.5.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. Child Care Facility Handbook Page | 17 A. All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. Documentation by the mechanic must be maintained in the vehicle. B. The maximum number of individuals transported in a vehicle may not exceed the manufacturers designated seating capacity or the number of factory installed seat belts. C. When transporting children, staff-to-child ratios must be maintained at all times. The driver may be included in the staff-to-child ratio if he or she meets screening and training requirements. D. All child care facilities must comply with the insurance requirements found in Section 316.615(4), F.S. Documentation shall be maintained at the facility. E. The interior of vehicles, when being used to transport children, must be maintained at a temperature comfortable to children (between 65- and 82- degrees Fahrenheit). The vehicle can be ventilated either by mechanical or natural means to maintain the temperature. F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare . 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above
