Then a second person went into the room after the licensing specialist asked them for another person to be in the classroom. This was during nap time. This is being marked completed at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.3.1 A. Mixed Age Groups A. During the inspection, Licensing Specialist observed on the playground the wooden fence on the far left of the playground by starting by the tree there was colored nails sticking out. The nails was in the section of the fence that was closer to the top of the fence. The director was given until September 23, 2024, to come into compliance with this standard. During the inspection, Licensing Specialist observed on the playground the mulch under the swing sets measured at 2 inches, jungle gym (red, blue, yellow and green) measured at 3 inches, jungle gym (yellow slide) measured at 3 inches , platform that is blue and green measured at 5 inches. The director was given until September 23, 2024, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.12.D.1- Outdoor Equipment. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. Child Care Facility Handbook Page | 39 1. In the rear corner of the infant/1-year old playground, there is a gap that is greater than 3 1/2" where the chainlink fencing meets the exterior/perimeter wood fencing. There was a small foldable toddler slide pushed up into the corner observed today in an effort to close this gap

04-01 · Supervision
Child 1 was on the playground in their class group doing an activity. Child 1 left the group and went to the infant/ones play area. Child 1 lifted the gate latch to open the gate and enter the area. There was one teacher present in the infant/ones play area. Teacher 1 acknowledged to CCRC and Manatee CPI that she was aware he had entered the area. Teacher 1 stated that the child does this often and since their class only had 3 children it was okay for Child 1 to be in there, per ratio. Teacher 1 stated they turned their back to attended to another child and when Teacher 1 went back to monitoring all children, Child 1 was not in the play area anymore. Teacher 1 stated they thought Child 1 went back to their group activity on the other side of the playground. Child 1 did not go back to the activity on the other side of the playground. Child 1 went out of the side gate of the infant/ones play area, unsupervised. Child 1 walked from the far end of the property(outside the gate) where the infant/ones play area was, all the way to the parking lot. A parent pulling in to pick up their child noticed Child 1 walking around the parking lot and took him inside the facility and told them they found him in the parking lot. It is unsure the amount of time that Child 1 was unsupervised outside the facility in the parking lot. The Director said they were not present at the time of the incident. However, CCRC observed that the director did fix the gate and locked it with a better lock that is child proof. TA: Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location

39-03 · Accident/ Incident Notification and Documentation
CCRC observed that there was an incident report written and signed by parent on the day of incident referenced in complaint. However, CCRC observed that there was no incident report for the incident that happened a week prior to the complaint. The incident a week prior; Child 1 tried to leave the infant/ones play area through the side gate, a teacher stopped him before he was able to get out of the gate. Parent was never notified of the incident of Child 1 trying to leave the area. TA: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur

46-07 · Daily Attendance
Child 1 was on the playground left their class group and went to the infant/ones class group without either teacher knowing. Both attendance logs were not accurate in the matter that Child 1 was not signed out of one class and signed into the other. CCRC did check attendance records at time of inspection and during inspection they were all accurate. TA: Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the childcare personnel and the group of children throughout the day should they leave the classroom
