During the inspection, Licensing Specialist observed in the 4 year old room there is a crack in the corner wall by the exit sign. In the 1 year old room there is a large hole in the wall on the right side of the sink with paint peeling. Both doors at the bottom of the 1 year old's room have holes in them. The director brought this standard back in compliance at the time of inspection by covering the holes with shelves or contact paper. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

During the inspection, Licensing Specialist observed in the infant room in the freezer there was 1 bag of breast milk that was not labeled with the childs first and last name. The director brought this standard back in compliance at the time of inspection by discarding the breast milk. During the time of the inspection, the Licensing Specialist monitored 6 of the 6 employee records. When the Licensing Specialist was reviewing the files the Licensing Specialist observed 3 personnel did not finish their 5 hours Early Literacy training. During the time of inspection, the Director advised the Licensing Specialist that there are 6 employees. During the time of the inspection, the Licensing Specialist monitored 6 of the 6 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. Specialist reviewed 10 hours of annual in-service, completed, for all child care personnel for the 2023/2024 fiscal year. When the Licensing Specialist was reviewing the files the licensing specialist observed three personnel did not complete their School Readiness training within the 90 days. The director was given until May 7, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, School Readiness Program Health and Safety Standards Handbook Section 3.1 Preservice. Child care personnel must successfully complete the preservice training coursework described in this section, as applicable. All child care personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. At the time of inspection, licensing specialist was let in the building by the director who was one of the four teachers supervising the mixed age group of 34 children leaving the group of 34 children with only three teachers. The youngest children were 5 one year old children. Licensing specialist observed and photographed children leaving classroom to move to another without any teachers. At the time of inspection licensing specialist observed and photographed an infant not capable of rolling over sleeping on her back. TA When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician.
