33-01 · Accident/Incident Reports Complete (Articles XIII.B and XV.E) (Articles XIII.B and XV.E)

The facility failed to complete a written record of an unusual incident. The facility received an email at 7:23PM on 7/10/24. The next day the parent and child came into the facility to verify the event had occurred with Staff Member A. At that time, an incident report should have been recorded and the parent's signature obtained in addition to pertinent staff. A Corrective Action Plan shall be written by management and a training shall be completed by all staff on how to document an incident/accident report. Management will need to also submit proof of all staff members who received this training to the Department on the date below. Re-Inspection Date: 7/22/2024
