Staff member A was unaware that a child had been left unattended in the hallway of the facility as they returned to the classroom from the playground. The children were counted by the staff member as they entered into the building, however, child A did not keep up with the group as they entered the classroom. Child A was approximately 2 feet away from the classroom door and the teacher who was standing inside the doorway, held the door open for the children not realizing that Child A had not made it into the classroom. ( Per Video Review) Management shall submit a Corrective Action Plan to address Direct Supervision Procedures when exiting the playground and returning to the classroom. Evidence of what staff members who took the training shall be submitted to the Department. Due Date: 5/22/2024
