The Department reviewed the video on 1/19/2024-1/23/2024. The Video footage takes place in the toddler/infant classroom. On the other side of the classroom was the infant class with two teacher and two babies. During breakfast time , The children had ate breakfast and two out five students were eating breakfast at the table. The rest of the children were seated on the carpet. As the video continue to play , Ms Douglas the toddler teacher was cleaning the table where the two children were eating and begin sweeping the floor . Once the floor was swept , Ms Douglas approach the carpet and begin yelling at (KL) , telling the child to sit down . The child begin to cry and she tell the child to "Hush your mouth". Ms Douglas approach the student and begin hitting the child (KL) 4 times. While she was hitting that student the other two teacher in the infant classroom located on opposite of the class was changing diapers/ assisting to other children . Ms Douglas grab the child by her arm and walked her over to be seated in time out. Once Ms Douglas seated the child she walked over to carpet and observed the child (KI) taking off her sock . she yelled out "Stop It" and pulled the sock away and hit (KI) and told her to "Shut her mouth" . As the video continues, the children are on the carpet and the child (MH) was playing in the cubby hole and Ms Douglas went to carpet pulled him out of the cubby hole and hit him on the hand three times. Also (MH) was seated on the carpet and she walked over and hit hand 3x times, Ms Douglas walked away and stated "Hush your mouth Little boy". Next, Ms Douglas is seen sweeping the floor , she stop sweeping and begins fussing at a (MH) and slightly picked him off his feet and dropped him in the seat at the table. The child (KI ) approach the table where the child (ML) was seated Ms Douglas told (KI) to go back to the carpet KI threw the two toys on the carpet Ms Douglas approach the child and told her to sit down and hit her on her hand four times and the child begins to cry . Ms Douglas walks away and begins cleaning again , The child (KI) approaches her while cleaning the rolling cart and Ms Douglas tells (KI) to go sit down the child walks to carpet and looks around to see if anyone is watching and the child goes to carpet and pulls her pants down and begins to squat. Ms Douglas looks near the carpet and notice the child (KI) pants is down and walks over and as she was pulling her pants up she begins to hit the child 4x times on the butt and she states to the child " Leave your pants alone and stop taking your clothes off" she place the child on the carpet and walks away and the child (KI) begins to cry. Lastly Ms Sheena Willis , peeks her head into the infant/toddler classroom and ask to speak with Ms Vixamary and Douglas about what she observed on camera. Once she spoke with them she spoke with Ms Earley to ask if she observed or heard anything while in the classroom

The following 3 children (KI), and (MH) (KL) parents were not notified the day the incident occurred. The video footage showed that the child (KI) was spank due to pulling her pants down, pull off her sock and throwing two toys on the carpet. According to (KI) incident report it states that staff member (JD) popped (KI) on her right thigh using her hand. There was no marks or bruises. The parent signed two incident on 1/18/2024 1/19/2024. The child (MH) was sitting on the carpet and she hit the child on the hand 3x times and pulled (MH) aggressively jerk him and place him on the left side of the carpet. Also (ML) crawling in the cubby hole and the teacher pulled him out and begin to hit him. The incident report stated that the staff member (DL) aggressively grabbed (ML) by the arm and place him on the floor. (MH) parent signed the incident report on 1/23/2024. Furthermore, the student (KL ) was sitting on the carpet and the staff member (JD) bent over and begin to hit (KL) and placed her in time out . The parent of (KL) was provided with an incident report that states that staff member used her hand to to pop child, there was no injuries observed. The parent signed the incident report on 1/23/2024

The provider contacted the Department of Health on 1/18/2024 at 11: 42 am to report an incident in the 1 year old classroom. Per Director , (AD) stated, she reported the first incident to the Abuse Hotline on 1/18/2024 after watching part of the video and no came out she then inquired to call DCF again to follow up on the case, and the case was closed. On 1/23/2024 she reviewed the full video footage. Once the video footage was reviewed . The Director stated she spoke with DCF to follow up on the first case but DCF informed her that the information was confidential
