Even though security protocols were in place a two-year-old child was able to exit the building via two doors that were supposed to be locked, but were not secured, two additional doors that are always unsecured, as well as a broken porch gate that was missing a lock. Provider will write up a staff memo outlining supervision and security protocols ensuring that staff are familiar with what must be done without fail to keep the children safe at all times. This staff memo will be signed by each staff member after reading, and one copy will be given to staff, while another will be kept in each staff member's personnel file. Provider will also send a friendly reminder to parents that classroom and playground doors need to be re-locked any time they walk through them to ensure the safety of all the children in care. Provider will email a copy of the memo including all staff signatures and a copy of the parent reminder to the licensing office on or before 5/26/2025

12-02 · Facility Environment

The lock on the porch gate in front of the building has been broken for months. A child was able to leave the facility and exit into the parking lot without adult supervision due to the broken lock. Provider will fix the broken lock on the porch gate and email a picture of the repaired lock/gate to the licensing office on or before 5/26/2025
