At the time of the inspection, the licensing specialist reviewed all staff files and determined the staff listed on the supplemental had an expired Child Abuse & Neglect form in their file. The staff signed one while the licensing specialist was present placing the facility back into compliance for this standard. The provider was reminded this form must be signed annually on or before the prior year's date

At the time of the complaint inspection, video surveillance footage revealed two teachers interacted poorly with two different students. One teacher slapped a table several times and made a child cry as redirection to get her from climbing on a table; claiming to prevent her from hurting herself. The second teacher yanked an autistic child by the arms and slammed her down onto the chair during lunchtime because the child threw her plate of the table. The second child had superficial bruises/marks on her legs. The noncompliance was resolved because both teachers were immediately terminated from the facility

At the time of the inspection, there were 9 ceiling tiles with water damage in the Blue Horse classroom, 1 ceiling tile in the Baby Bop classroom, 6 ceiling tiles in the Blue Clues classroom and 1 ceiling tile in the Pooh Bear classroom. TA was provided that all these ceiling tiles that are water damaged will need to be replaced
