One or more children were not adequately supervised in that [a child was able to escape the facility through a door that automatically unlocks with movement and the child pushed the door open and left the facility and was found on the side of a busy road which posed an imminent threat to a child, and could or did result in death or serious harm to the health, safety, or well-being of a child. The child was found outside near the road after 3 minutes and 58 seconds of being outside per video. Completed at time of inspection as a corrective action was implemented
