A complaint was received on March 21, 2022, regarding lack of supervision, not providing an accident/injury report, and concerns regarding not following the center's suspension/termination policies. An investigation was started on March 29, 2022. There is no evidence to substantiate this complaint because staff stated they observed the interactions between the children and called Child #2 back into the cafeteria when she attempted to leave. Staff stated no injury report was written because there were no injuries on March 15, 2022, but the suspension notice noted what took place and Child #2 s parent refused to sign it. The center stated they have followed their suspension/termination policies based on their code of conduct. Two concerns were noted. The first is that one staff member was observed on his cell phone instead of actively engaging with the children in care. The second is that 22 children were present and only 21 children were signed in.
Disposition: No Evidence to Substantiate with Concern
On August 3, 2021, the Office of Child Care Licensing received a complaint alleging lack of supervision, inappropriate discipline, and failure to report. An investigation was conducted on August 9, 2021. This complaint is substantiated for inappropriate discipline and failure to report because a child stated a staff member hit her and another child observed the staff member raising her hand back in the air in what appeared to be getting ready to hit a child. The staff member also yelled at the child. According to the regional director, when asked about the incident, Staff #2 stated I did not hit her with any force. Additionally, a report of the allegation against the staff member was not called into the Child Abuse Report Line or OCCL until August 3, 2021, although the incident happened on July 29, 2021. There is no evidence to substantiate this complaint for lack of supervision as the parent left the children outside of the center after being told the children could not be accepted into care. Staff #1 visually supervised the children while at the doorway until she received direction to take the children into the facility.
