Specialist noticed the exit gate on the older kids playground was bent and latches on both playgrounds were broken. Mulch on both playgrounds needs to be replenished.
Ensure child files are complete. Child #2 and #4 need blood lead screening results.

Ensure staff-to-child ratios are maintained at all times through the assignment of qualified staff present with the children. An aide who has been employed for more than one year was alone with three children, the youngest being an infant, in the infant classroom. This was corrected when another staff was placed into the classroom.

The playground needs more resilient material in the fall zones. The vinyl siding outside of the Toddler classroom on the playground needs to be repaired or replaced. The self-latching mechanism between the playgrounds needs to be repaired or replaced. Ensure shade is available on the playground.

Ensure children's attendance is documented with the correct arrival and departure times. In the pre-k class, there were 11 children present, but only 10 was signed in.
Ensure all areas are clean, safe, and in good condition. Floors and carpets throughout the building needs to be cleaned and sanitized.

Ensure anywhere will children feet leave the ground is protected by a fall-zone. Playground needs additional fall-zones. LS Smith observed the gates on the fences locked. This was corrected when they were unlocked, and the center agreed to remain compliant.

Ensure lunches brought from home are properly stored. LS Smith observed lunches in students cubbies, with cold items that needed to be refrigerated. This was discussed and corrected when the teachers put all lunched in the fridge.
Ensure pets are kept clean and sanitary. Dirty fish tank in the school-age classroom.
Ensure nap equipment is labeled. Ensure nap equipment is in good condition, with no rips or tears. Ensure children who are 12-18 months and not walking sleep in a crib or pack-and-play. Staff disclosed that they had a 11 month old who is not walking sleeping on a mat.
Ensure all classroom have soap and water, and bleach bottle solutions. School-age classroom was missing both solutions.
Ensure mops are cleaned and sanitized daily, and hung to dry. LS Smith observed dirty mops, and dirty mop water sitting in all classrooms.
Ensure bottles and cups are labeled with the child's name.
Ensure children are not exposed to any hazards, and all hazardous materials are stored inaccessible to children. There is a hold in the ceiling in the School-age classroom. There is exposed drywall in the pre-k and pre-school classroom. There was detergent and other cleaning materials on the floor in the school-age classroom., which was moved into a closet.
Ensure staff files are complete. Staff #2, 3#, #4, #5, #6, #7, #8, #9, #10, #11, and #12 need updated professional development plans. staff #8 needs annual review of safe sleep, and review of abuse and neglect. Staff #12 needs annual review of safe sleep, review of abuse and neglect, orientation, employee declaration, drug and alcohol statement, release of employment, updated service letters, and to be re-fingerprinted.
Ensure all infants enrolled have a written feeding schedule, and that it is updated monthly.
Ensure all staff attendance is documented with the correct arrival and departure times. Staff #9 and 10 , in the toddler classroom, had sign in an out times prefilled for the day. Staff #10 and 6 was physically present in the toddler room, but not signed in on the tracking sheet.
Ensure child files are complete. Child #9 needs date of enrollment and the days/hours they attend care. Child #4 and 9 need an emergency card. Child #1 needs a health appraisal. Child #2 and 3 need health appraisals and immunization record. Child # 2 and 3 need Parent Right to Know forms. Child #2 and 3 need screen time permission. Child #4 and 9 need emergency transportation permission. Child #2 needs updated infant feeding schedule. Child #3 needs mat permission.
Ensure all areas are cleaned and in safe condition. Air vents throughout the building, including bathroom ventilations, need to be cleaned. The ceiling tile in the boy's school-age bathroom needs to be repaired. Ensure that all garbage cans used for trash have a cover. The toddler room's trashcan needs ac over.
Ensure all food provided is free from spoilage. LS Smith observed opened fruit sitting on the counter, vegetables sitting on the stove, and chicken nuggets in the oven on the warm setting. All food was in danger of reaching un-safe temperatures. LS Smith also observed children's lunches were sitting in their cubbies with items that needed to be kept cold. The lunches were in danger of reaching an un-safe temperature. This was discussed and corrected when LS Smith talked to the cook and administrator about checking children's lunches when they arrive. As well as not leaving food sitting out in the kitchen. The children's lunches were put away in the fridge, and in the kitchen was properly stored. Ensure that all food stored is properly labeled with contents and date of expiration. The food in the fridge and two freezers were observed to be not labeled properly.
Ensure all fences have self-closing and self latching abilities. Self-closing and sel0latching attachments must be installed to all gates.
Ensure first aid kit is complete. First aid kit is missing an eye patch/pad.
Ensure evacuation route diagrams are posted in each classroom. The pre-school room and the toddler room needs an evacuation route posted. Ensure there is enough emergency water and food to sustain all staff and children currently enrolled for 24 hours.

Staff #5 needs eight additional training hours, with three out of the eight being in health and safety. Staff #6 needs nine additional training hours ,and a PD plan. Staff #7 needs a PD plan, annual review of child abuse and neglect, orientation, job description, two references, employee declaration, drug and alcohol statement, health appraisal with TB results, and a service letter. Staff #8 needs fifteen additional training hours, a PD plan, annual review of safe sleep and child abuse and neglect, two references, employee declaration, drug and alcohol statement, health appraisal with TB results, release of employment history, and service letters. Staff #1, 2, 3, 4, 5, 6, 7, 8, 9, 10, and 11 need first aid and CPR training. Staff #2, 5, and 6 needs to be trained in administration of medication.
Ensure proper staff/child ratios are maintained at all times. Staff #1 has not been determined eligible by the criminal history unit, and was left alone supervising the infant classroom. This was corrected when a qualified staff member entered the classroom to supervise.
