On August 31, 2022, The Office of Child Care Licensing received a complaint regarding a lack of supervision and improper discipline. This incident had previously been self-reported to OCCL by the facility administrator. An investigation was conducted on August 31, 2022, and September 7, 2022. Based on the results of this investigation this complaint is substantiated because LS Flowers and Budin observed video footage of Staff #1 picking up a child by the arm. There is no evidence that Staff #1 was not properly supervising the children because LS Flowers and Budin observed video footage of Staff #1 seeing one child fall on top of another child and immediately attending to the children. Non-compliances from this incident for improper discipline and failure to notify a parent immediately of a potentially serious injury were cited in a self-reported event on August 31, 2022.

On June 10, 2022, the Office of Child Care Licensing received a complaint regarding lack of supervision. An investigation was conducted on June 10, 2022, and June 15, 2022. Based on the results of this investigation, this complaint is substantiated because LS Flowers and Budin observed Staff #1 filling out paperwork while Child #1 had feces in her hands for several minutes.

Ensure all children have current MAR forms , including rash creams and sunscreen. Send forms for rooms 3, 4, and infants.
Ensure all lesson plans are posted & available for review.
Ensure all air fresheners and plug-in air fresheners are not used in the classrooms. Remove from classrooms.
Ensure all cabinets used to store cleaning products and hazardous materials are locked or out of reach of children.
Clean pillows/covers in rooms 3 & 4, dramatic play cozy area.
Ensure floors, walls, and surfaces ae cleanable. Remove peeling tape from the floor in Room #4. This was corrected at the visit.
Ensure center has an adequate emergency water supply.
Ensure that all staff use developmentally-appropriate methods of behavior management of children. Staff #1 roughly handled a child when she picked the child up by the arm. Staff #1 must complete the following three hours of professional development training which can be found on the DEPDNOW website: Positive Behavior Management. Submit documentation of completion to OCCL by September 30, 2022.
Ensure all serious or potentially serious injury, a licensee shall ensure the parent or guardian is notified immediately after staff members have assisted the child. This was corrected when LS Flowers counseled the administration team on this regulation.
Ensure staff attendance is accurate. At the time of the visit, two staff members in room #10 were not signed into the classroom.
Ensure children's attendance is accurate. At the time of the visit, three children left the classroom with a staff member and were not signed out.
Ensure that all staff use developmentally-appropriate methods of behavior management of children. Staff #1 admitted to striking her own child while the child was enrolled at the center and in care at the facility. This was corrected when Staff #1's employment was immediately terminated from employment and the center owner contacted the Child Abuse and Neglect Reporting Hotline.
