On August 5, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision and not having proper security procedures for the entrance. An investigation was conducted on August 9, 2024. This complaint is substantiated for lack of supervision based on an interview conducted. Administrator #1 stated Child #1 was left alone in the office with no direct supervision. This complaint is substantiated for not having proper security procedures for the entrance based on an interview conducted noting there is an occasional issue of the door not latching properly when parents close it. Administrator #1 stated if it is closed properly, it locks automatically.

Ensure the Medication Administration Record information is complete. Obtain MAR log for Child #21. Send completed log to OCCL by October 14th, 2022.
