On November 24 , 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to provide an injury report to the parent within one business day. An investigation was conducted on December 1, 2025. There is no evidence to substantiate this complaint for failing to provide an injury report to the parent within one business day. An additional concern noted is a head injury was not communicated to the parent immediately after assisting Child #1.
Disposition: No Evidence to Substantiate with Concern
On August 12, 2025, an incident of inappropriate parent behavior between a parent and the provider was self-reported by Provider #1 to OCCL. On August 15, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to provide clean bedding as required, not being respectful while speaking to children and others, failing to identify the ongoing needs of the child and learn parent or guardian preferences, goals, concerns, or special circumstances that may influence the child's development, behavior, and learning, and failing to follow release of children policy. An investigation was begun on August 12, 2025, and concluded on August 21, 2025. There is no evidence to substantiate this complaint. LS #1 observed clean bedding for all children in care. Based on interviews conducted and statements received, Provider #1 followed the policy on releasing children, was respectful when speaking to children and others, as well as meeting the needs of the children in care while learning the preferences of the parents.
