The Office of Child Care Licensing (OCCL) received a complaint on June 22, 2026, regarding concerns of operating over staff-to-child ratios and not using respectful communication amongst staff. An investigation was conducted on June 26, 2026. This complaint is substantiated for improper staff-to-child ratios based on staff interviews and review of attendance records. The opening classroom operated over the maximum ratio and group size for a room that has three-year-olds present. On June 22, 2026, there were 44 children present; on June 25, 2026, there were 39 children present, and on June 26, 2026, there were 41 children present. Additionally, review of attendance records showed the school-age classroom over staff-to-child ratio with 16 children present with one staff member on June 24, 2026, and June 25, 2026. There is no evidence to substantiate this complaint for not using respectful communication among staff based on interviews conducted. Of seven staff members interviewed, five staff noted no concerns with administrative staff, stating they felt supported and had no concerns. One staff member stated they felt more support from one administrator over another, and another staff member stated they had a previous incident where they felt Administrator #1 was hostile, but it has since been resolved. An additional concern noted during this investigation was Staff #8 working alone in the four-year-old classroom during nap time without a DEEDS Early Learning certificate. Another concern noted is in addition to exceeding maximum staff-to-child ratio and maximum group size in the opening classroom, the facility also exceeded the maximum capacity for the measured space. This classroom is measured for only 18 children but had up to 44 children at one time based on attendance records and interviews.

On May 10, 2024, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 10, 2024, and accepted it for investigation regarding improper discipline, failure to report suspected abuse or neglect, failure to notify parents of a significant incident to a child, and not following termination policies. An investigation was conducted on May 21, 2024. This complaint is substantiated for improper discipline because five of the staff members interviewed stated they had witnessed Staff #2 improperly discipline children through verbal threats as well as physical contact and provided multiple detailed instances. This complaint is substantiated for failure to report suspected abuse or neglect because multiple staff stated they had witnessed improper discipline by Staff #2 and did not report the incidents to the Child Abuse and Neglect Report Line. Multiple staff also stated they had brought their concerns regarding Staff #2 improperly disciplining children to Administrator Erin Goldberg and Ms. Goldberg did not report the incidents to the Child Abuse and Neglect Report Line. This complaint is substantiated for failure to notify parents of a significant incident to a child because Ms. Goldberg stated she did not contact Child #1 or Child #2 s parent or guardian about the incidents because Staff #1 or Staff #2 was not present at the center on the day of the allegations, and she could not prove anything. However, the attendance documentation collected at the visit shows Staff #1 and Staff #2 were both present and working in classrooms on the morning of the allegations. This complaint is unsubstantiated for not following employment termination policies because Staff #1 was on her documented final warning and was terminated due to her aggressive behavior towards other staff members. Both incident reports regarding Staff #1 s term ination were collected at the visit.

The Office of Child Care Licensing (OCCL) received a complaint on November 09, 2022, regarding staff not following procedures to notify a parent of important information relating to a child. An investigation was conducted on November 21, 2022, and January 13, 2023. The complaint is substantiated because a child informed two staff that another child pulled his pants down, and neither staff notified the child s parent(s) of the incident. The administrator also confirmed the staff did not inform the center s administration and instructed them to inform her of such occurrences in the future. However, although they did not witness the incident, the administrator reported that the two staff notified the parent of the child who reportedly pulled down another child s pants to prevent a repetition of the alleged behavior.

The Office of Child Care Licensing (OCCL) received a complaint on January 18, 2023, regarding lack of supervision and staff not having an understanding of and respect for a child and their needs. An investigation was conducted on January 20, 2023. Based on the video footage of the incident, this complaint is substantiated for lack of supervision. A child entered the three-year-old classroom from the four-year-old classroom to gather his belongings for nap time, and neither of the two staff in the four-year-old classroom accompanied the child or informed the staff member in the three-year-old classroom that the child was coming over. The administrator and a staff member, who was in the four-year-old classroom when the incident happened, stated a staff member stood at the doorway to supervise the child as he gathered his belongings. However, the video footage did not show a staff member supervising the child at the doorway. If a staff member had been standing in the doorway, they would have seen the door close and heard the child s cries for help before the staff member in the three-year-old classroom heard him since she was in the kitchen area and unaware the child had entered the classroom. There is no evidence to substantiate the allegation of staff not having an understanding of and respect for a child and their needs because the staff member in the three-year-old classroom assisted the child as soon as she heard him asking for help.

On February 14, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio, infant feeding requirements, and improper care of infants. An investigation was conducted on February 15, 2023. This complaint is substantiated for improper care of infants because the staff interviewed stated diapers are checked every two hours when they are required to be checked at least every hour. This complaint is substantiated for infant feeding requirements because the administrator stated staff admitted to sharing their own food with the infants and cereal was observed in an infant s bottle. There is no evidence to substantiate the complaint for improper staff-to-child ratio because attendance records and video footage showed proper staff-to-child ratio was maintained. Additional concerns noted at the time of the visit were: infant monthly feeding schedules were not completed by parents of five children, infant bottles were not labeled, infant daily logs were not accurate, and staff was observed multiple times using a cell phone on video footage while caring for infants.

On July 12, 2022, a complaint was called into the Child Abuse Report Line and not accepted for investigation by the Institutional Abuse Unit. The Office of Child Care Licensing (OCCL) received the complaint on July 12, 2022, and accepted it for investigation on July 13, 2022, regarding lack of supervision. An investigation was conducted on July 15, 2022. This complaint is substantiated because two toddlers, ages one and two, were able to exit the outdoor play area unnoticed by two staff, and were observed by the reporter on the side of a high traffic road in front of the facility. One of the children ran into the middle of the road and the reporter brought the child over to the grass on the side of the road. The two staff received written warnings for lack of supervision. The facility implemented a new policy allowing only the administrative staff to open and close the gates in the outdoor play area, and to turn on/off the water hose.

On December 2, 2021, OCCL received a complaint regarding Lil Einstein s III regarding improper discipline. An investigation was conducted on December 7, 2021. This complaint is substantiated because Staff #1 was observed in person and on video yanking a toy from a child and improperly picking up the child. There is no evidence to substantiate an accident report was not provided because the center has no record that the child was kicked in the face by another child.
