Accurate attendance with exact time in and out must be maintained. Two children were signed in and out for the day of the visit and had not attended. Send attendance sheets for the week of April 10 and 17 to OCCL on Friday, April 21, 2023.
One enrolled and the provider's child were observed in the child care space while the provider was outside talking with a neighbor. Ms. Webb was addressed and agreed to always have children in direct supervision without exception.

On June 28 and 29, 2022, OCCL received two complaints regarding FCCH provider Lakisha Webb regarding lack of supervision. An investigation was initiated on June 30, 2022, by phone and an investigation visit was conducted on July 7, 2022. Based on the results of the investigation, this complaint is substantiated because Ms. Webb left a three-year-old child unattended in the park for at least 45 minutes. Ms. Webb left the park and did not notice the child was not with her. Other non-compliance items were also noted during the visit because attendance records were not complete, Ms. Webb was over capacity by having four additional children, and Ms. Webb provided conflicting information to the police officer who returned the child and to OCCL.

Attendance records for children must state exact times of arrival and departure and be recorded at those times. Ms. Webb s attendance was not completed each day and no times were recorded. Provide July s attendance record to OCCL by Monday, August 1, 2022. Submit a written plan indicating how the provider intends to assure accuracy with attendance.
A level one FCCH may have up to five preschool-age children present or four preschool-age and two school-age children. Ms. Webb had six preschool-age and younger children in care with four under 24 months of age. Adhere to the schedule for daytime and evening care created to correct the non-compliance in the July 18, 2022 complaint. Capacity may not be exceeded at any time for both the total number and age of the children. Send a revised schedule to OCCL by July 29, 2022.
