Additionally, this incident was self-reported by the facility on January 5, 2024. An investigation was conducted via telephone and email beginning January 5, 2024. An investigation was conducted on December 19, 2023. There is no evidence to substantiate this complaint for failure to use standard precautions to prevent illness based on staff interviews conducted. Staff members interviewed stated they follow all precautions to prevent illness in the facility. Staff #1 unintentionally touched lips with Child #1 as she picked him up during drop-off. Staff #1 stated she learned of an exposure to COVID-19 but tested negative prior to coming into the center and had no symptoms at any time. There is no evidence to substantiate this complaint for concerns about the information written on an accident/injury report based on the interviews conducted and the documents collected. Parent #1 was notified immediately of Child #1 s injury via the center s mobile application. The initial written accident/injury report provided to Parent #1 did not include all details of how Child #1 s injury occurred but did not provide any false information. The initial report just stated Child #2 scratched trying to get the book Child #1 had. Administrator Irene Kerrigan re-wrote the accident/injury report to include more information and provided it to Parent #1 via the center's mobile application. Staff members interviewed stated families are notified of all accidents/injuries by pick-up time each day but are notified sooner if an injury is more severe. Incident reports were viewed for Child #1 incident and were signed by the parents.

There is no evidence to substantiate this complaint for not following the facility's policy regarding notification to parents of significant incidents or injuries based on the interviews conducted and the documents collected. Staff members stated families are notified of all injuries and incidents by pick-up time each day but are notified sooner if an injury is more severe. Incident reports were viewed regarding the behavior of the child biting other children and the observed reports were signed by the parents. There is no evidence to substantiate this complaint for lack of supervision based on interviews conducted and attendance records collected. Staff members stated classrooms are always in ratio and attendance records supported this for the dates these biting incidents occurred.

The Office of Child Care Licensing (OCCL) received a complaint on July 3, 2023, inappropriate interactions with children in care. An investigation was conducted on July 11, 2023. There is no evidence to substantiate this complaint based on staff interviews conducted. All five staff members interviewed denied witnessing or participating in speaking inappropriately to the children in care. Staff members stated they have not observed Staff #1 speaking loudly or using harsh tones with the children.

This complaint is substantiated for inadequate diaper changing because the reporter stated Child #1 still had feces on his bottom after a diaper change and Staff #1 acknowledged she changed him while standing up, instead of on a diaper-changing table, and may have missed something.

A complaint was received on April 20, 2022, regarding not modeling appropriate behavior, incorrect staff-to-child ratios, and not documenting children s attendance accurately. An investigation was started on May 3, 2022. This complaint is substantiated because a staff member was speaking inappropriately in the presence of children, the toddler classroom had one staff member and at least 6 children, with the youngest being an infant, present at one time, and staff and child attendance was not accurately documented.
