A licensee shall ensure supervision of children is provided at all times. On October 29, 2025, Staff #1 left Child #1 in the classroom for 16 minutes when Staff #1 joined Staff #2 outside. As a correction, Staff #1 received a warning of termination with a probation period.

On May 11, 2023, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 11, 2023, and accepted it for investigation on May 12, 2023, regarding improper discipline. An investigation was conducted on May 16, 2023. This complaint is substantiated for improper discipline because the staff member told the administrator and another staff member that she pushed the child because he scratched her. The center immediately terminated the staff member.
