On March 24, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding not following procedure for documentation and notification of accident and injuries. An investigation began on March 23, 2026, and concluded on March 26, 2026. Based on a self-report, interviews, and documentation reviewed this complaint is substantiated for not following procedures for documentation and notification of accident and injuries. There were no written accident or injury reports present in Child #1 s file.

On March 5, 2024, a complaint was called into the Child Abuse and Neglect Report line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on March 6, 2024, and accepted it for investigation regarding improper discipline. Additional information was received on March 7, 2024, regarding similar behaviors involving the same staff member. An unannounced visit was conducted on March 7, 2024. The complaint is substantiated because the staff members who were interviewed confirmed the observations stated by the reporters. The center acted promptly by conducting an internal investigation and terminated the employment of Staff #1 effective March 8, 2024.
