On April 20, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding using improper behavior supports. An investigation began on April 21, 2026, and concluded on April 23, 2026. Based on video footage and staff interviews, this complaint is substantiated for using improper behavior supports. In the video footage, Staff #1 was observed grabbing two children s wrists, with one being more aggressive than the other, and guiding them down to the floor where one child was visibly upset and crying. Administrator #1 also stated this action was observed to be aggressive. Staff #1 was put on administrative leave on April 21, 2026, and received a written warning on April 27, 2026. Staff #1 returned to work on April 28, 2026.

On October 14, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding not following policy for handling parent complaints, staff not being respectful when speaking and interacting with others at the center, and staff not respecting children and their needs. An investigation was conducted on October 16, 2025. Based on documentation and staff interviews, this complaint is substantiated for not respecting children and their needs. All staff interviewed stated Staff #1 had three children clean vomit from the floor, which was not appropriate. There was no evidence to substantiate this complaint for not following policy for handling parental complaints and staff not being respectful when speaking and interacting with others at the center. Staff interviewed stated parent concerns were addressed promptly, and no staff members were harassing others or being disrespectful to others.

On June 3, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect staff-to-child ratios, incorrect spacing of sleeping equipment, mold present in indoor spaces, holes in walls, unsafe heating/cooling equipment, and the administrator not communicating with staff or using disrespectful communication. An investigation was started on June 4, 2025, and concluded on June 10, 2025. This complaint is substantiated for incorrect staff-to-child ratios. A review of attendance records revealed multiple instances where there were incorrect staff-to-child ratios. This complaint is substantiated for incorrect spacing of sleeping equipment as incorrect spacing was observed in two classrooms at the time of the unannounced visit. This complaint is substantiated for unsafe cooling equipment as Administrator #1 stated they had portable air conditioning units in two classrooms for at least two weeks without notifying OCCL or receiving a variance approval. Children had potential access to these units that could pose a risk to safety. These units were removed on June 6, 2025. There is no evidence to substantiate the complaint for the presence of mold in indoor space or holes in the walls. No mold or holes in the walls were observed during the unannounced visit and all staff interviewed stated they had not observed mold or holes in the walls. There is no evidence to substantiate the administrator not communicating with staff or using disrespectful communication. Based on staff interviews, the majority of the staff interviewed did not state communication was disrespectful by the administration, but two staff did have some concerns regarding administrator communication. An additional concern noted was there were four classrooms that exceeded measured room capacity at the time of the unannounced visit. Another concern noted was staff and child attendance were not accurate as there were many instances where both staff and children were not signed out on at tendance records reviewed.

On March 24, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. This incident was previously self-reported to OCCL by the facility on March 19, 2025. An investigation began on March 19, 2025, and concluded on March 27, 2025. This complaint is substantiated for improper discipline based on staff interviews and documentation received. Both staff interviewed stated Staff #1 put Child #1 on their cot not gently and, in a written statement, Staff #1 admitted to putting Child #1 down way harder than [Staff #1] should have. Staff #1 resigned their position as of March 19, 2025.

On March 12, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of supervision. This concern had been self-reported to OCCL by the facility on March 11, 2025. An investigation was conducted on March 19, 2025. Based on the results of the investigation and staff interviews, this complaint is substantiated for a lack of supervision. The staff did not complete the proper name-to-face transition set in place by the facility, leaving Child #1 on the playground for two minutes. The administration also completed their own investigation. Staff #1 was terminated from employment effective March 14, 2025, and Staff #2 resigned effective March 19, 2025.

The Office of Child Care Licensing (OCCL) received a complaint on November 7, 2022, regarding improper staff-to-child ratio, improper discipline, diapering procedures, and handbook policies. An investigation began on November 16, 2022, and ended on December 8, 2022. The complaint is substantiated for improper discipline because Staff #1 admitted to holding Child #1 s hands behind his back when Child #1 slipped off her lap to attempt to get away. There is no evidence to substantiate the complaint for not following diaper procedures, because Staff #3 knew diapers needed to be changed every two hours. There is no evidence to substantiate the complaint for improper staff-to-child ratio, as the center s attendance records for two weeks prior to the received complaint documented correct staff-to-child ratio. There is no evidence to substantiate the complaint for violation of handbook policies, as LS Catka confirmed the center followed their policies regarding termination of children from care.

The Office of Child Care Licensing received a complaint on November 21, 2022, regarding disrespectful communication with a staff member, and by a staff member. An investigation began on December 1, 2022, and ended on December 22, 2022. The complaint is substantiated for disrespectful communication with staff, because two staff and Ms. Dries acknowledged Ms. Dries use of profanity when talking to Staff #2. Staff #2 was disciplined for disrespectful communication with the children in her class. A concern noted was three staff reported a child had been left on the playground for a short time, in September 2022, by Staff #2. Staff #2 was disciplined and her employment subsequently terminated.
